How do I make an appointment at HMC?

 

To schedule an appointment as a new or existing patient, please follow these steps:

 

Step 1:


Call us to schedule an appointment. We are open Tuesdays from 10:00am until 4:00pm, on Wednesdays from Noon until 7pm, and on Thursdays from 1pm to 4pm. To contact HMC call: (303) 776-7117

 

Step 2:


When you call, we will schedule a registration appointment and a medical appointment for you.

All new patients or patients who have not received an updated financial rating in one year or more are required to complete a registration visit prior to their medical appointment. This can be done immediately before your appointment or on another day.

 

Step 3:


On the day of your enrollment appointment, please bring the following documentation:

Hopelight is a charitable 501(c)3 medical clinic that serves low-income patients in Longmont and the surrounding areas. We accept Medicaid, Medicare, and the Child Health Plan Plus Program(CHP+). We also provide an extremely low cost sliding scale for all other patients, with rates from a maximum of $20 to $0.

 

HMC Sliding Fee Scale Discount Program (SFS)

 

Hopelight offers access to quality health care for patients who don’t have insurance or don’t have enough insurance through a sliding fee discount program, with discounts available, based on your current household size and income. The Program can cover you for up to 12 months—a new application is required yearly or when your household size and income changes, whichever is sooner.

 

This program allows you to see Doctors at Hopelight Medical Clinic and offers access to lab, x-ray and prescriptions at a huge discount. If you receive medical services from someone other than your Hopelight provider, you will be responsible for the full cost of that visit.

 

Once you are approved under the Sliding Fee Scale Discount Program, you’ll be issued an ID Card stating that you are a patient of HMC. This is not an insurance card and it cannot be presented as such to outside of the Hopelight Network. Call Hopelight to schedule a registration appointment today!

How do I cancel an appointment at HMC?

Please contact us at (303) 776-7117 to cancel your appointment. Please let us know as soon as posible so that your appointment time may be given to someone else who needs care.

 

We do take walk-ins but every patient will need to complete the registration process before they can visit the medical clinic.
