How do I make an appointment at HMC?

 

To schedule an appointment as a new or existing patient, please follow these steps:

 

Step 1:


Call us to schedule an appointment. We are open Monday, 9am to 3pm, Tuesdays from 9:00am until 3:00pm, on Wednesdays from 9am until 3pm, on Thursdays from 9am to 7pm and on Fridays from 9am to 3pm. To contact HMC call: (303) 776-7117

 

Step 2:


When you call, we will schedule a medical appointment for you. All new patients are asked to come in a few minutes early so that you can fill out our registration forms.

Hopelight is a nonprofit 501(c)3 medical clinic that serves patients of all incomes, primarily in Boulder, Larimer, and Weld counties. We accept Medicaid and Medicare. We also provide an extremely low cost sliding scale for uninsured patients.

HMC Sliding Fee Scale Discount Program (SFS)

 

Hopelight offers access to quality health care for patients who don’t have insurance or don’t have enough insurance through a sliding fee discount program, with discounts available, based on your current household size and income. A new application is required when your household size and income changes.

 

This program allows you to see Doctors at Hopelight Medical Clinic and offers access to lab, x-ray and prescriptions at a huge discount. If you receive medical services from someone other than your Hopelight provider, you will be responsible for the full cost of that visit.

How do I cancel an appointment at HMC?

Please contact us at (303) 776-7117 to cancel your appointment. Please let us know as soon as posible so that your appointment time may be given to someone else who needs care.

 

We do take walk-ins but every patient will need to complete the registration process before they can visit the medical clinic.
